10/8/2023 EnchantaVaughn Approved Behavior: QMHP-A greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a positive mood evident by her facial expressions and overall conversation with QMHP-A. QMHP-A engaged Ms. Brown in a discussion surrounding her afternoon. Ms. Brown expressed to QMHP-A that her afternoon was going well. QMHP-A informed Ms. Brown that this session was going to continue to focus on her improving her IDAL’s and ADL’s being as though it is beginning to get hot outside and summer is approaching. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHP-A began the session by inquiring about Ms. Brown’s current thoughts, feelings and emotions. QMHP-A inquired about Ms. Brown’s mental stability and engaged Ms. Brown in a discussion surrounding her medication compliance. QMHP-A educated Ms. Brown on independent living skills and being able to function as a normal adult. QMHP-A discussed with Ms. Brown about following daily living skills such as complying with medication regiments, bathing, changing clothes and hygiene regiments. QMHP-A encouraged Ms. Brown in maintaining organization throughout the community and home. QMHP-A discussed with Ms. Brown the importance of personal hygiene regiments and how it will improve her living skills when completed daily. QMHP-A reviewed with Ms. Brown cleaning regiments such as washing clothes, separating clean clothes from dirty clothes and organizing her area in her room (getting hot to prevent rodents and other insects from entering the home). QMHP-A engaged Ms. Brown in a skills building activity. QMHP-A explained to Ms. Brown that the purpose of this activity was to demonstrated proper cleaning techniques through role play. Response: Ms. Brown appeared to be receptive to QMHP-As’ interventions and services that were provided to her during the session. Ms. Brown expressed to QMHP-A that she struggles with her independent living skills because she doesn’t live on her own. Ms. Brown expressed to QMHP-A that she really didn’t know a lot of independent living skills until she came connected with QMHP-A. She discussed with QMHP-A her mental health has a lot to do with it. Ms. Brown was quiet when asked about personal hygiene. Ms. Brown admitted does not practice proper hygiene regiments daily; however, she is a work in progress with it.
